I am fully aware that this program contains a martial arts component that involves rolling, tumbling, falling,
grappling, and other physical contact. That despite careful and proper preparation and instruction, not all
hazards and dangers associated with the activities can be foreseen, and there is still a risk of injury.

I agree to indemnify and hold harmless Art of Noyz MINISTRIES and each of their instructors, teachers,
officers, directors, employees, contractors, volunteers, members, agents, and assigns from or against any
and all claims made or instituted against it or them arising out of the acts of myself or my child
participating in the Non-Violent Conflict Resolution and Walking In the Art of Noyz Conflict Resolution
Martial Arts sessions sponsored by Art of Noyz MINISTRIES , while using their equipment for the
purpose of practice or demonstration, including injury or loss by myself or my child however caused and
injury or loss caused by myself or my child to any other person while engaging in our virtual training
sessions . I understand that it is always recommended that a person see a doctor prior to starting any
physical activities.

I understand that the Virtual Art of Noyz CONFLICT RESOLUTION Martial Arts Program documents at its
sole discretion activities and events involving classes and field trips, and I freely give Art of Noyz
MINISTRIES permission to use any documentation written, audio, and visual, including but not limited to
notes, video tape and digital recordings, photography, digital imagery, negatives, reproductions, audio
recordings, or film in which me or my child’s image or likeness is taken in whatever way Art of Noyz
MINISTRIES wishes. I further agree that any media whether film or digital images or recording may not
be distributed or posted on or off the internet without prior consent from Art of Noyz MINISTRIES. I
understand that Art of Noyz MINISTRIES is the sole owner of this documentation.

I, also agree to indemnify and hold harmless Art of Noyz MINISTRIES and their instructors, teachers,
officers, directors, employees, contractors, volunteers, members, agents, and assign from and against
any and all claims, demands, actions, complaints, suits or other forms of liability that shall arise out of or
reason of, or be caused by the use of me or my child’s creative work(s), photograph, likeness or voice on
television, radio or motion pictures, or in the print medium, or on the internet or any other electronic/digital
medium.

Be it further understood, that I agree that no monies or other consideration in any form, including
reimbursement for any expenses incurred by me or my child, will become due to me, my child, our heirs,
agents, or assigns at any time because of me or my child’s participation in any of the above activities or
the above-described use of me or my child’s photograph, likeness, or voice. I certify by my signature that I
have read and understand this Agreement in its entirety and all my questions have been fully answered.
