AON Ministries WAIVER RELEASE FORM for Art of NOYZ Annual Basketball Classic

(1) I am aware that this is a 3 on 3 basketball tournament, 3point shootout, and Free throw Contest. The activities will be played at @ Julian High school Saturday August 28,2021. All players are responsible for transportation to games.

(2) Parents must send lunch/snacks with your child if they have particular foods they must eat. (It is your responsibility to advise us of allergies or medical conditions.)

have had a recent physical, been cleared by A physician, and does not have physical limitations or illness that would cause them harm while participating.

(4) I am aware that I / my child __ (initial and circle) will be performing such strenuous physical activities such as running, jumping, stretching and basketball games. I am aware that I am responsible for consulting my / my child's physician before my child engages in any physical activity

(5) All volunteers from Julian Highschool, Art of NOYZ Ministries Inc. Partners and volunteers are exempt from any liability regarding this basketball tournament.

(6) I am aware that there will be photos and videos taken of this camp by the sponsor’s tournament. I give my permission for Julian Highschool Officials, Art of NOYZ Ministries Inc and volunteers to use photos of my child for general promotional purposes only. Ie. Brochures, websites or marketing literature. No monies will be paid for the promotional uses of said photos.

I/We the individual playing or parent of individual printed below are participating in the Art of NOYZ Annual Basketball Classic and hereby give my approval to the participation in any and all of the activities during this event. I/We, assume all risks and hazards incidental to the conduct of the activities, transportation to and from activities. I/ We, do further hereby release, absolve, indemnity and hold harmless, Julian Highschool , Art of NOYZ Ministries Inc., the organizers, sponsors, members, officers, their heirs, any or all of them. I/We hereby likewise release from responsibility any person transporting my son or daughter to and from the activity. In case of injury to son, daughter, or myself I/We hereby waive all claims against Julian Highschool, Art of NOYZ Ministries Inc., the organizers, sponsors, members, officers, their heirs, any or all of them.
